Sharing Incidents and Near MissesWe want to bring safety even more into
focus at this year's conference by drawing on your experiences.
A refiner at the 2003 seminar had the courage to talk about their $7 Million fire and give an evaluation of the incident, root cause analysis and lessons learned. You can bet that all of us took those cautions home with us. Here's from another fellow coker
